One thing is for sure, there will be a huge twist in the game no one sees coming.

He could be referring to the scale of the game and how gigantic it is compared to the others. I'm sure he meant that cause he also said that Kojima keeps a tight lid on the story. No one really knows what's really going on. It's a need to know basis for voice actors.
