We see the Phantom infiltrating what appears to be a normal, real-world casino, but in the gameplay we see him fighting demons in said casino.

Not a whole lot, apparently. He did the boxarts and some promo art, and going by the illustration credits given for Hatsune Miku he also did at least the character render for Miku, but possibly the whole cast. He was also resposible for Kanami's design and most likely the side characters.

There's a second charecter designer listed in the credits, so he probably didn't do all the side characters.
Masatoshi Kobayashi and Hanako Uchibe are also listed alongside him as charecter designs, so it's likely that they did the side characters.

Yeah, there's bokeh during the scenes where the supernatural happens, but I don't know if that's a metaphorical effect or if that's just Atlus trying for some stylized cinematography. It doesn't just happen at the edges - it's also happening everywhere outside the depth of field in the second image, making me think that they can't use the effect as much outside of pre-rendered cutscenes.This might be nothing, but if you look at the trailers there's this sort of bubbly particle effect around the screen when the characters are wearing their costumes that could indicate some kind of detachment from the 'normal' world.
It could be sort of like the scanline effect in P4 when the characters were in the TV World.
