There just has to be a reason the medics face was intentionally not shown in GZ. Seriously the camera made SURE not to reveal anything above the mouth. Also it was Sutherlands voice just in a different pitch.

Kojima doesn't do anything for no reason. The man is a stickler for detail. There is a reason for this.
Not to mention his face model is literally the same as Big Boss's, just without all the extra hair and a younger skin texture.

Honestly, I believe kojima is a horrible write who is hell bent on ruining every good character he has ever made.

Solid Snake ends up a fuckin old dude...

Liquid Snake, one of the best villains in all of gaming gets strung out in some stupid arm/hypnosis bullshit.

Otokan had to fuck his step mom or whatever.

Raiden was shit from the get go and he tried way too hard to salvage.

And if the spoilers are true, 5 is going to ruin big boss as a character.

Raiden had sex with his mom over a decade before the start of MGS2. Please don't reference this Kojima. 5 is cements Big Boss as GOAT so fuck all that shit you talking about. Big Boss raised TWO armies by himself and convinced the world, according to rumors, that his body double was a villain. Big Boss is the GOAT after TPP whereas Solid Snake is the boss with 1/4 the talent.
 
That's why I believe the medic isn't a nobody. His face is probably obscured because we would recognize him. Why else would it be?

All info has been confirmed with the guide leak.

Skull face origins and history with Big Boss
-His parents worked in a factory for a secret army
-Recruited in secret by Major Zero, his job with XOF? Make sure that the important missions didn't fail, (including Operation Snake Eater),
-Working for Zero for years lead to him resenting the two of them, that being Big Boss and Zero ofc

-Skullface has the parasite, which was made by Code Talker(who also has the cure), infects Zero with it at some point, Big Boss has to kill all of MB due to it spreading. Yongyea called it.

Bosses
Quiet
-You face quiet in a "dual of sharpshooters" in mission 11. And mission 40 on extreme difficulty.
-Her hearing is ten times more sensitive than normal soldiers.
-Her vision is unaffected by time of day, no difference in sight between night and day
-Something involving a laser sight kills the player instantly
Quiet leaves after mission 43.
She's heard speaking during the hospital segment, she's there to kill Big Boss.

Volgin AKA Fireman
-Other than the prologue, the only time we fight Fireman is during mission 20.
-One of the missions is about retrieving his body

Eli
-You'll face Eli during Mission 23
-There's a brief fight during optional mission 113.

Skulls
There are three types of skulls.
Fog Version
-You will meet him twice. In missions 1 & 6.
-You can destroy his body parts to rapidly reduce their health and resistance.
-Health slowly regenerates overtime, (all skulls have this ability).

Armor Version
-Encountered during missions 16 & 29
-If you fulton this type of skull, you'll be rewarded with their parasite. Enhanced armor.
-Every type of skull can be fultoned.

Camouflage version
-Encountered during mission 28 and mission 48.
-Fultoning them and gaining their parasite leads to the creation of items that grant the player 30 seconds on invisibility.
-This version's body parts can't be destroyed.
-Headshots are the most useful way to defeat these skulls
-Like Quiet they have perfect vision during the night time. They share her hearing abilities as well.
-You can hear their breathing if you listen closely while they're aiming with their sniper rifles.

The "floating kid" aka child psycho mantis controlling Metal Gear is the final boss.

General Info
If you spray Quiet with water she will "wiggle" because the parasites inside her are absorbing water. She also reacts to rain, the same thing occurs with the skulls,
There are 50 main missions, 157 side missions

Glad to see Extreme difficulty confirmed. It's one of the most basic things in an MGS game but there was some serious radio silence with respect to choosing difficulty.
It is possible that the guide is referring to later encounters as "extreme difficulty' since it shows that they have much larger health pools during those missions. Or it could be referring to hard mode. Or you're right and extreme difficulty changes enemy places and bosses show up in different levels.

Yeah it might be predetermined without any way to manually choose. Like the first X number are Normal, next Y on hard, last Z on Extreme, e.t.c.
 
Quick question about the character creator, the name you put, is that displayed online, or is it you tag. ? Does anybody know.

First off, the newest Metal Gear from the last trailer and the guide is so sick. Looks like Jehutey with a Rex head. Soo Awesome!

What isn't imo though, is playing another entire Metal Gear game as someone other than Snake. If we find out TPP's main playable character is a look alike or someone made too look like Snake I'm going to be throughly disappointed. Especially since this is the last game. Solidus is one thing, but some random medic! Wtf.

Your source is still the italian guide, right? In the italian version of Ground Zeroes, hard mode is called "Estrema"(=Extreme) :(

Big Boss/Ahab

- The guide strongly suggests through clues that the Medic from Ground Zeroes is the version of Big Boss who awakens from the coma in the hospital. This version of Big Boss is know as Ahab.

- The guide notes that Big Boss and the Medic have similar voices, so he is a strong candidate for being the version of Big Boss/Ahab that awakens from the coma in hospital.

- The guide also says that the Big Boss we see on the operating table in the "Don't you die on me dammit!" scene from the trailers does not have shrapnel in his head. The guide suggests that the Medic is the person to mostly have the shrapnel in his head, while also being the same hospital room, as he protected Big Boss from the Paz's bomb blast in the chopper in Ground Zeroes.

- The guide says that the version of Big Boss/Ahab that awakens for the coma in hospital has a DNA test done with Eli. Their DNA does not match. The guide goes on to state that Eli is a clone of the real Big Boss, their DNA should match, which again gives a glue to Ahab's identity.

- The guide doesn't say who Ishmael definitely say who Ishmeal is. Konami and Kojima Production asked Piggyback, the creators of the guide, not ti reveal it. Ishmael identity is revealed in an optional mission after the main story has ended.

- Huey is not sure of the version of Big Boss/Ahab's identity when he first meets him.

- It does however, go over some fan theories and gives clues towards the true identity of Ishmael.

- The guide discusses the theory that Ishmael is imaginary. It says that this can't be the case as Quiet sees Ishmael when she comes to assassinate the version of Big Boss/Venom Snake/Ahab that awakens for the coma in hospital.

Meanwhile, in Italy:

You may not like the idea that we'll be playing as some unnamed-medic-turned-Big-Boss-decoy (can't say I was too fond of it either back when it first came up), but it sure looks like it's what's going to happen.

It isn't speculation. They just aren't allowed to flat-out say it because Konami wanted it that way. What they can do, apparently, is hint at things.

As much as I hate the idea of not playing as Big Boss I'm excited to see all the craziness unfold. Seems like Kojima finally out Kojima'd himself.

It says "it's safe to assume". Keep in mind that the page that this is taken from is pretty the last page in the guide, so it's still trying to not directly spoil the story for the reader.

And just to clarify, in terms of the identity of the version of Big Boss/Ahab of that awakens from the coma in the hospital, the Medic and Ishmael, the guide says that they are giving clues.

It is not "speculation" or suggestions", they are clues :-)

None of that makes him a worse character, it makes him a better, more believable character. Complaints 2 and 3 are just absurd. So he got some help to achieve some goals? Yep, now he's worthless! /s
And the first point is just an origin story. Instead of just knowing/inventing CQC and being a sharpshooter in the cradle, he was trained by and then surpassed a master.

Of course, a lot of this will be undone if we don't actually play as BB in MGS V.

From the sound of it fultoning them gives you their parasites for special items like invisibility and enhanced armor.
