If this really isn't Snake then how is he able to fight and infiltrate places as well as Big Boss? If this is true it would give a lot more weight to what Ocelot says to him in the opening, when he tells him about how he's a legend and needs to live up to it.

Skull face giving Zero the parasite causing him to make JD and the rest of the AIs actually seems like it makes sense in this context.

I'm speculating that the Boss was also cloned or atleast attempted by Zero after Operation Snake Eater. After seeing what the Sarrow was capable of it would make a lot of sense if Psycho Mantis was a failed clone of the Boss. There's also Paz and her twin seen in TPP trailers that look a hell of a lot like her.

I like a lot of these ideas if they do become true but I'm still going to be disappointed if my last tour with Snake is actually just a decoy.

I'm really confused...

Who is the medic? What is the significance of the medic for him to become BB?

And also, then where is the real BB and what happened to him?)

Oh so this is the one from the 2015 E3 trailer with the zombies.

She is holding a sniper rifle in one of the guide scans and one skull is in heavy armor and another one is holding a sword.

Do you actually fight them all togther on each battle?
The guide says that they appear in different missions, it appears that multiple of each type will appear in one mission. Quiet is most likely a camouflage type since the abilities, down to the hearing and sight of their AI, are exactly the same.

Yep, this all checks out with what has come from the guide.

I agree completely. All the rumors and supposed leaks sound great to me, except that we're not actually playing as BB. Hope for sure that one is wrong. Everything else sounds pretty freaking cool to me.

"Hey, we need someone to dub that completely irrelevant unnamed character... I know, let's use the most expensive member of our cast! You know, to save money! And then, we'll probably do the same for the Japanese version. Coincidentally, of course."

It's three lines. Kiefer is in the booth. Yes, it is more economical for them to use Kiefer in that instance.
Meanwhile, in Italy:
"Apparently, we're not allowed to flat-out reveal the big final twist... But I guess we can publish some hints... that point to some completely false conclusion! Because that makes sense, right? Right."

You may not like the idea that we'll be playing as some unnamed-medic-turned-Big-Boss-decoy (can't say I was too fond of it either back when it first came up), but it sure looks like it's what's going to happen.
You're acting like a twist regarding BB's identity is the ONLY twist that can happen. It's pure confirmation bias.

I'm really confused...

Who is the medic? What is the significance of the medic for him to become BB?

And also, then where is the real BB and what happened to him?)

A nameless dude who has three lines voiced by Kiefer at a lower pitch who Kojima conceals in a cutscene that everyone has convinced themselves is more important than any other soldier who dies at Mother Base.

Medic is from GZ. Who he is, is unknown. Guide says he is BB's most trusted man. Also watching that GZ ending again, I just realized (after reading the guide of course) that the Medic DID block/shield most of the explosion from BB. He got the most of the damage. Kinda crazy I never noticed that til now.

Nah, he's already ruined Big Boss via character dismemberment.

The character defining traits of Big Boss have been surgically removed and transplanted into other characters:

Big Boss was the greatest soldier ever because he just was. Nope, The Boss taught him everything.

Big Boss created and led a (the first?) PMC. Nope, Miller is the one that pushes him into it and runs mostly everything.

Big Boss creates a nation for soldiers, of soldiers. Nope, that was Gene's idea.
Amen. He just gets lamer with each new game. MGSV is pretty much the last chance to salvage what's left of the character's originality.

Some of these rumors sound a little promising though. I was totally against the idea of a Big Boss double, however if the point of the twist is to demonstrate how willfully callous and manipulative he can become when driven far enough, then it could work as a satisfying callback to character traits established from the MSX games.

The Medic is Big Boss's most trusted guy. He has gone through reconstructive surgery and all that MGS science shit to be able to emulate the real Big Boss on the field.

Literally who else could it be?

The Medic shares half a name with Para-Medic, who has to come back onto the scene at some point to mess with Frank Jaeger and make him a cyborg. Dr. Clark confirmed

Which has nothing to do with MGSV. S3 wasn't designed until after Shadow Moses as a simulator recreating the events of the original mission.
Python has a video that went into this topic and how it could link up in the timeline I think it can make sense.

This isn't gonna be another Kaz situation is it, where we're introduced to an important character in-between games, "most trusted man?" Who?

I hope Para-Medic or zero show up in this game, i mean comon kojima we need to see them at least once after the Les Enfants Terribles project! MGS4 doesnt count since zero was literally a corpse.

Big Boss really trusts too many people if you think about it.

Kaz
Ocelot
Gray Fox
Medic
Chico/Quiet

No wonder his base got blown up.
You forgot the one guy who's probably holds the responsibility for that.

Forget Big Boss becoming a monstrous supervillain, I want to see Sigint's fall to becoming the DARPA Chief.

At the very least we should definitely get to see Zero in the flesh again. It would help make the whole feud between him and Big Boss feel a bit more tangible/relatable.

I watched the Ground Zeroes ending again and Big Boss doesn't have any blood on his face.

One of the trailers (don't you die on me dammit/ Not your kind of people) shows him with a bloody face in the GZ outfit while looking out a chopper. Wonder what that's all about.

Can someone link me to spoilers please? I went a few pages back but couldn't find anything.
.
Ok so this is what I have so far, adding new info overtime.

All info has been confirmed with the guide leak.

Skull face origins and history with Big Boss
-His parents worked in a factory for a secret army
-Recruited in secret by Major Zero, his job with XOF? Make sure that the important missions didn't fail, (including Operation Snake Eater),
-Working for Zero for years lead to him resenting the two of them, that being Big Boss and Zero ofc
-Skullface has the parasite, which was made by Code Talker(who also has the cure), infects Zero with it at some point, Big Boss has to kill all of MB due to it spreading. Yongyea called it.

Bosses

Quiet
-You face quiet in a "dual of sharpshooters" in mission 11. And mission 40 on extreme difficulty.
-Her hearing is ten times more sensitive than normal soldiers.
-Her vision is unaffected by time of day, no difference in sight between night and day
-Something involving a laser sight kills the player instantly
Quiet leaves after mission 43.
She's heard speaking during the hospital segment, she's there to kill Big Boss.
Volgin AKA Fireman
-Other than the prologue, the only time we fight Fireman is during mission 20.
-One of the missions is about retrieving his body

Eli
-You'll face Eli during Mission 23
-There's a brief fight during optional mission 113.

Skulls
There are three types of skulls.
Fog Version
-You will meet him twice. In missions 1 & 6.
-You can destroy his body parts to rapidly reduce their health and resistance.
-Health slowly regenerates overtime, (all skulls have this ability).
Armor Version
-Encountered during missions 16 & 29
-If you fulton this type of skull, you'll be rewarded with their parasite. Enhanced armor.
-Every type of skull can be fultoned.
Camouflage version
-Encountered during mission 28 and mission 48.
-Fultoning them and gaining their parasite leads to the creation of items that grant the player 30 seconds on invisibility.
-This version's body parts can't be destroyed.
-Headshots are the most useful way to defeat these skulls
-Like Quiet they have perfect vision during the night time. They share her hearing abilities as well.
-You can hear their breathing if you listen closely while they're aiming with their sniper rifles.

The "floating kid" aka child psycho mantis controlling Metal Gear is the final boss.

General Info
If you spray Quiet with water she will "wiggle" because the parasites inside her are absorbing water. She also reacts to rain, the same thing occurs with the skulls,
There are 50 main missions, 157 side missions
Just to refresh, the guide states the following points.

This information is taken from these pictures:

A video proving the validity of the these pictures can also be seen here:

I am semi-fluent in Italian and have personally translated these points myself from the pictures in the link.

Other native Italian speakers have also verified these points.

General

- Avatar creation is for the online elements only.

- The are 50 main missions.

- Quiet leaves you after mission 43 and to finish her story you will have to find her after the main one is over.

- The last mission is against the Metal Gear controlled by "The Third Child".

- Quiet goes to the hospital to kill Big Boss and speaks while doing so.

- The "man on fire" is Volgin. You have to get recover his body in Chapter 2.

- The guide says it's safe to assume that Eli is Liquid Snake.

- The alternate name of the ST-84 Metal Gear is confirmed to be the Sahelathropus.

Big Boss/Ahab

- The guide strongly suggests through clues that the Medic from Ground Zeroes is the version of Big Boss who awakens from the coma in the hospital. This version of Big Boss is know as Ahab.

- The guide notes that Big Boss and the Medic have similar voices, so he is a strong candidate for being the version of Big Boss/Ahab that awakens from the coma in hospital.

- The guide also says that the Big Boss we see on the operating table in the "Don't you die on me dammit!" scene from the trailers does not have shrapnel in his head. The guide suggests that the Medic is the person to mostly have the shrapnel in his head, while also being the same hospital room, as he protected Big Boss from the Paz's bomb blast in the chopper in Ground Zeroes.

- The guide says that the version of Big Boss/Ahab that awakens for the coma in hospital has a DNA test done with Eli. Their DNA does not match. The guide goes on to state that Eli is a clone of the real Big Boss, their DNA should match, which again gives a glue to Ahab's identity.

- The guide notes that when Eli asks Ocelot where his father is, Ocelot replies that "Your father isn't here". This is despite the fact that Ahab is watching their conversation behind a two-way mirror.

Ishmael

- The guide doesn't say who Ishmael definitely say who Ishmeal is. Konami and Kojima Production asked Piggyback, the creators of the guide, not ti reveal it. Ishmael identity is revealed in an optional mission after the main story has ended.

- Huey is not sure of the version of Big Boss/Ahab's identity when he first meets him.

- It does however, go over some fan theories and gives clues towards the true identity of Ishmael.

- The guide discusses the theory that Ishmael is imaginary. It says that this can't be the case as Quiet sees Ishmael when she comes to assassinate the version of Big Boss/Venom Snake/Ahab that awakens for the coma in hospital.

- The guide says that the fact that there are "The Star of Bethlehem" lilies in the Ishmael's and the version of Big Boss/Ahab that awakens for the coma's room is a strong clue to the real identity of Ishmael. The states that these were The Boss' were hold a significance to The Boss.

- When Ahab sees the ghost of Paz the first time at Mother Base, he has an hallucination where there are clearly two versions of Big Boss on the chopper.

Music Tapes

The map from the guide also confirms that these are some of the music tapes that are collectable from different locations. These are additional to some of the other music tapes that have already been confirmed:

So if Quiet talks during the hospital scene, I wonder if it's pre-infection by the parasite, and her being infected is to help her chances of getting him the 2nd time around?

We need to pick sides.

There are post-game missions yes? So you still continue to play after the story is finished? If you were just some random clone, you'd think you would probably be killed off at the end. Though if you are in fact Gray Fox, you would obviously live through the story. But maybe you play as the real Big Boss after the game is over?

"The guide states that at the end of The Phantom Pain, the parasite that mutates peoples' vocal chords infects Mother Base. This forces Big Boss to exterminate his soldiers to prevent a global catastrophe."

Shrug.

i wonder if it happens in a cutscenes or gameplay, i dont know if can handle this.

I don't believe ANY of this information circulating the web. I think it's utter bullshit. Plain and simply, the character we play as is the real Big Boss. Most of what I've read falls very short of anything remotely logical or practical to the plot. If we see Gray Fox in this game, he will be an entirely separate character that we encounter -- not who we've been playing as.

You don't believe ANY information? The photos and videos of the guide are fake?

The info is pretty solid about now.

The story information came from the official Phantom Pain guide published by Piggyback. Unless the information inside it is intentionally false, it's pretty much irrefutable.

Also, the excerpts in the guide at no point state that any possible versions of Big Boss in the game are Grey Fox.

I'm guessing the "vocal cord parasite" is Skullface's big plan. Maybe it makes it so where no one can speak a comprehensible language. It ties in well with the whole "Language" trailer.

There's no reason to believe Grey Fox is in the game at all, that's pure speculation, but there's lots of evidence to point to us not being Big Boss at this point. As for it not having anything to do with the plot, well...

I went back and watched the E3 2015 trailer earlier, and the two opening dialogues, one by Skull Face and one presumably about Skull Face, struck me differently based on the spoilers we've heard.

"I was born in a small village. I was still a child when we were raided by soldiers - foreign soldiers. Torn from my elders, I was made to speak their language. With each new post, my masters changed, along with the words they made me speak. With each change, I changed too. My thoughts, personality, how I saw right and wrong. Words can kill."

"Time and again the country was ruled by a foreign tongue. When he was a young boy, he lost his native language, the bedrock for any developing child. His country, his family, his face, his identity - everything was stolen from him."

If the leaks are correct Venom Snake is a protagonist whose had his own identity stripped away and forced to be someone else, and the idea of a parasite that destroys vocal cords ties in very clearly to the theme of the importance of language.
