It felt like the editor cut to black in the middle of the scene. It didn't feel like an ending at all. It just left me befuddled. The episode should have ended with Rick walking towards Tyrese to help him bury Bob. The shot was great.

The scene where team Rick hacks Gareth and friends to pieces while Glenn and Maggie stood in awe and watched spelled it all. I predict that while Judith represents Ricks last moral shred of decency, her presence being in danger could equally tip him over the edge...He'll likely have to revisit Daryl's council(for lack of a better term) I think in the coming episodes to remain grounded. Will he even stay with the group though when Carol insists on herself parting? Who the hell knows!!! Oh yeah, the Bobby-Q was a nice touch but it wouldn't matter in the long run if they ate him because the disease s already inside them all...I can't believe Maggie and Glenn left! The only other "big" city relatively close by would be Columbus. Interesting. Wonder if it really might be the same hospital Rick was at.They travel very slowly on foot throughout the show and are constantly sidetracked by people getting seperated or hurt, so it's safe to say they're still close to Atlanta.
And looking at the Terminus map that was used at Comic Con... Terminus appears to have been located in or near Macon.

You're right.Well I believe the town that Rick, Lori, Shane, and Morgan are from is fictional but it was a rural town, not in Atlanta proper. The hospital Rick was at when he awoke was in his hometown and then he traveled to Atlanta in search of Lori and Carl. You might remember the Iconic image of Rick on the highway that lead into Atlanta.
The Hospital could be anywhere really. Be it a real or fictional city. However if turns out to be in Atlanta, it wouldn't be the same one that he woke up in.

It was more the repeated post kill stabbings than the killing.
Probably had to do with how gruesome it was. Killing with a gun? Totally OK. Killing with a close combat weapon? OMG, we're not human anymore. But yeah, it probably had to do with how inhumane it looked.
