How are the stages being chosen in Melee?

There is a set of 5 stages for the first match, the players strike stages until one is left in this order:

P1-P2-P2-P1

For the next matches one stage is added to the list, the loser chose the stage. You can't choose a stage where you have already won unless the opponent agrees to.
Winner is allowed to change character as long as he does before the loser. (Since a stage can counter a character, you would not want to be in disavantage against stage and character in a match)
