I see a lot of questions about things easily presented in the manga, and at first I'm like "wtf, did you not read it?"

But Kubo has a way of switching back and forth with over exposition and then just empty ass chapters, so people speed read because he doesn't know what pacing is.

I honestly didn't recognize them. The book basically gave up on Ichigo having some kind of life like a job or school to worry about for years at a time. The last time we saw him doing anything besides saving the world was the start of the Fullbring Arc.

Chad being the only one looking older is probably due to his foreign facial features.

I think Rukia stayed the same "age", she just got longer hair.
