I'm guessing that floating orcish head is supposed to be one of those rotating floating enemies... or maybe one of those miniature goblin heads from Air Man's stage?
Either way it really puzzled me and creeped me out back then.

I think it is one of these two. It looks more like the first...but that one doesn't fly.
The second one does fly...but it doesn't really look like it.
