UC4 spoilers:
I nutted at least five times during the chase sequence in chapter 11. It felt a bit like a rehash of a similar sequence in UC3 but was much better executed.

Mostly just most eliminations in a short span and I think damage plays into it too. I once got one with mei but that was cause my freeze hit 5 people and I got credited for all their eliminations
Also revives appear equal to eliminations. You seem to need at least 3 of either to get it
I'd say better but I wouldnt mind being in a sandwich with them
