I just downloaded the Rome: Total Realism mod available for Rome: Total War. It kicks major ass. There are a billion changes that range from purely aesthetic to deep in the units and cities in the game. I've noticed quite a few changes with only a little of play time.
One of the changes that struck out at me is the phalanx formation. In Total War, if you charge a phalnx with heavy calvary, the formation will break. The calvary men will die, but the formation will break and be vunerable. Allot of people have argued that isn't realistic, that a calvary charge would not break the formation. In Total Realism, the formation will not break. I tested it out, and sure enough there is a clear difference. I think its a good change to show the phalanx as its commonly perceived today.
Aside from those types of changes, there is a huge amount of changes to the single player that make it quite a different game. There are new units, units taken away, changing the names of units and factions, all sorts of stuff added. I plan on starting a new campaign tonight.
There's also a slew of new historic battles like Spartacus VS Rome, and I think Caesar VS Pompei. Allot of work went into these historical battles, theres so many of them, and they are incredibly detailed.
Other changes are purely aesthetic, and they really are for the better. Changes to the correct Roman armor, correcting the Roman flag, new outfits for basically all the units. Here, check some of them out-

Its all pretty damn cool. Almost feels like a new game.
