When was the first time you felt 'TRULY' terrified in a game?

The most terrified I've been in a game isn't in the sense that you mean. It's playing a really difficult game where I'm going through a super difficult area where there hasn't been a save point in like an hour knowing if I die I'll lose everything and be forced to throw the game out the window. That's what terrifies me when I game.

I used to be a scaredy-cat to the point that I never tried any of the Resident Evils until two years after RE4 came out.
RE4 was so visually spectacular that I just had to finish it and I am glad I did because it gave me the courage to try so many other horror games.
with the exception of FEAR and CONDOMENED, nothing had scared me since, until ... PT was released.
I first tried the demo with my Sony Gold headset and 3D/surround audio.
the sheer terror that the first sighting of Lisa caused me, the sleepless nights that followed with me hopelessly trying to solve the messed-up final puzzle while reducing my life expectancy by years
was perhaps the most unforgettable memory I have ever had of a horror game.

Tell us about yours!
(kindly blur out any spoilers)
Probably the first time playing Quake (the original P.C. release) back in 1996. The fact that zombies, shamblers and fiends could chase you into even the most discrete hiding places and rip you to shreds. I was like 10-years old at that time but you mentioned the 1st time.

Resident evil remake on the GameCube, the bit with the sharks always freaked me out, and those crimson head zombies, you would crouch down to set them on fire, and they would be up and running.....

FFXI in Dynamis. We farmed Xarcabard for over a year before a Duelist's Chapeau dropped, one I was first for. I was playing on a shitty laptop with a shaky wifi connection. If I DCed at any point, it would have autolooted to another random person in the alliance. Also, it was a hot item to ninja loot because of how rare it was. Thankfully none of that happened.

Anyway, terrified.

It's difficult to say which game first gave me the chills because I've been a gamer since sprites were first introduced to TV screens. Back in the day, it was my own imagination that did all the work - even some text adventures could be terrifying!

Clive Barker's Undying was pretty scary at the time.

For whatever reason, I've grown away from horror games. I got really mad at RE5 because of how bad it was, and I think the only ones I've played since then are Amnesia DD and the Dead Space games, which are more jump/gore than horror to me.

First time hearing unsettling music:

A creature type that really disturbed me when I first saw it(and the first time I played the Silent Hill series):

First time I was truly terrified however, goes to P.T.

It genuinely makes me worried if I can handle future horror games as graphics get more and more realistic.

Being violated as a child by a Redead.
I have never been a fan of Zelda but it's interesting how often the name has come up, on this thread
Zelda devs must have been on something
they scarred so many kids

Easily PT, and I refuse to play any other horror games after that. I'm still scared of PT
it is incredible how a demo that is barely 30 minutes has had such an impact on horror fans, yours truly included

Zombies are a joke to me now but Resident Evil 2 on ps1 in 1999. I was 8 or 9 years old. Opening and beginning of the game terrified me and I had never used tank controls before so I just died first try. Had trouble sleeping for weeks.

I haven't really played Silent Hill except for a demo of the first game and I have Silent Hill 2 & 3 in my backlog. They seem pretty scary

The only way i can explain it is that Zelda on the N64 had a real dark and gritty vibe to it. Honestly i wish they would go back to that kind of theme as it was far more compelling than the more child-like experience and bright colors they do now.

back in the 90's/early 2000's, this was the stuff of nightmares. Also the music was unsettling.

Meh RE4 was never scary. Intense yes but that's about it. The actual horror game I played was Fatal Frame 1&2, but the one that is truly terrifying , it has to be PT. Replace this meme with Lisa

REmake.

Honorable mention...the first time a Redead freezes you in Zelda OOT. The sound effect combined with the sudden, unexpected loss of control seeing Link Frozen was brilliant design...invoking feelings in the player in a way only a game can achieve. I STILL remember the jump I got the first time that happened to me as a kid lol.

My buddy and I, in our twenties, squeeled like little girls when that pigman in Manhunt reved his chainsaw and came barreling after us. Too funny.

Yeah i honestly couldn't play pt, or visage. Two of the scariest fucking games ive ever seen. I had to watch people play them between my fingers. Fucking horrifying

I still to this day think they accidentally hired some horror artists on the dev team for the 64 games. Both of the 64 games are completely different in tone to every single other Legend of Zelda game that came before and after. From the dark and muddy textures everywhere, to the future having reddened skies surrounded by destroyed buildings and enemies reminding you of death. Even the overworld and NPCs during pre-time jump daytime felt oddly uninviting, regardless of the heroic music they would play while you traveled. There were areas that were supposed to be considered beautiful and safe like Lake Hylia that instead triggered thalassophobia as the music went silent, all you see is a single house, a bridge, no fish, and a dead tree, and you were afraid something would grab you or kill you the minute you entered the water.

Also like I said above, the weird items, oddities, and NPCs who just had this look of unease or at worst, looks that line up with the concept of horror, that would make even the nicest of towns like Kakariko feel off and unsettling.

While the NPCs in this blue sky town might seem festive in this at first, after exploring you start to notice the cracks, and then you notice how none of the buildings share the 'festive' nature of the town and look dark, ominous, and imposing instead. Going into the alleyways and corners break the illusion of what the town wants a new visitor to see vs what it really is.

Even things like this in the sequel:

Like why put this in the game knowing how horrifying this looks?

Silent Hill when i was 9. Shit had such an impact on me that i had real nightmares/halucinations when i had a very bad case of fever. Must have fried something in my brain as nothing ever scared me anymore.

Don't know if i should thank my dad or condemn him for letting me play the game.

Although i did feel some unease when playing RE VII on the PSVR. First playthrough too.

I've not really been scared by horror games, Dead Space was super tense at points for me I guess.

One time I can remember feeling very tense is when playing Dark Souls and walking into the Undead Parish (or church, whatever it was) and being pretty scared hearing that undead mage uttering spells from in the rafters on my headphones.
