Haven't gotten my GMG code yet....yet my brother who bought it after me has. Getting really annoyed now.
It's not in my list of games too, unless I'm looking in the wrong place. I log in and click into my account then click "Games" and it's blank. Opened a support ticket but it hasn't been answered yet.

They're included with the game, just have to wait for the game to unlock before you can access the bonuses.
Except in boss fights. If you use a Lifegem or even an Estus in any time except that one moment where it's safe (and those moments vary from rare to split-second precision timing rare, depending on the bosses), you're practically committing suicide.

It asks you if you're sure about joining until you realize you're about to make a huge mistake and decline.(Btw, it's not that bad actually...)
I've to try it in my game and see what kind of warning it give you.
Developers say in an interview that want to make covenant more "clear" without need to look on the internet but i think they completely miss their objective.

I'm just uploading the unmodified ~1GB video to mediafire. Note that it's 1080p downsampled from 2560x1440 with 4xSGSSAA, so it actually runs at a variable ~40-60 framerate.
