No missed up to stage 5, where I accidentally destroyed a lantern before I was close enough to get the bullet cancel. Made it up to Larsa's third phase before my second death, and my third death was a suicide for the endgame bonus. I made a few scoring mistakes here and there, so I think I could push 900+ if I had a "perfect" run with my current strategies (with 2 or 3 deaths on Larsa), but I'm pretty happy with this for now.
I see the world record is 1.5 billion... I have to assume the guy no missed no bombed the entire game to get such a ridiculous score. Makes my head hurt just thinking about it.

Speed has increased significantly from last night to tonight. The latest MAME build apparently runs the games at full speed (or close to it, I'm not sure if the slowdown is accurate.)
